A contractor died after a crane capsized at a refinery outside St. Louis, Illinois, the coroner said Tuesday.
Chad L. Crabtree, 47, of Owensboro, Kentucky, died Tuesday afternoon at the scene of the incident at the Phillips 66 Refinery in Roxana, 17 miles (27.4 km) northeast of St. Louis, said Madison County Coroner Stephen. Non said.
Crabtree was the project manager for refinery subcontractor Sterett Crane & Rigging, says Nonn.
During the operation of the crane, a mechanical malfunction occurred, causing the crane to capsize, Nong said.
Crabtree suffered a head injury, he said. An autopsy was scheduled for Wednesday.
A second man operating the crane was also injured and taken to hospital, Nong said.
Nong said the incident is still under investigation by the Roxana Police Department and workplace safety investigators.
